By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by THE SCHOOL. I/we agree to release and hold harmless THE SCHOOL, including its teachers, dancers, staff members and facilities used by THE SCHOOL from any cause of action, claims, or demands now and in the future. I/we will not hold THE SCHOOL liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behaviour in addition to any damage I/ we may cause to the facilities utilized by THE SCHOOL. I understand that THE SCHOOL is licensed, accredited and insured. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Executive Director, Artistic Director, Individual Instructor or staff member as soon as possible. Consent* Afterschool Terms and Conditions Participation and Dress Code 1. Only registered children may participate in the Afterschool Program activities and snacks. 1A. Snacks will be provided by The School. Do not send your child with snacks as we are a peanut and tree-nut free facility. 2. Students who are NOT registered in a program/class/workshop may not stay at The School for any extended period of time unless accompanied by parent/guardian. 4. Please ensure your child wears comfortable, clean clothing and indoor shoes. 5. Children will be given a designated bin to store their belongings. Please bring and leave a pair of indoor shoes and a water bottle. 6. Please do not send your child with toys from home. Pick-up and Drop-offs Walking Bus: Up to 2 (two) qualified and identifiable representatives from The School will pick-up children registered in the Afterschool Program at a specific spot at the participating local school as determined by The School Directors and the participating local school. Please ensure you have provided written consent prior to pick up to both THE SCHOOL CREATIVE ARTS and your child’s school. 1A. The Walking Bus will walk the children to The School snow, rain or shine. Please have your child dressed appropriately for the weather conditions. 2. Drop-off time for the Afterschool Program is 3:30 pm. We can not guarantee supervision or be held responsible for any child dropped off before 3:30 pm. 2A. Pick-up time for the Afterschool Program is 6:00 pm. We reserve the right to charge a fee of $10 per 15 mins for any child picked-up after 6:00 pm. 2B. Extended Pick-up: 2nd Class Option pick up time is at 6:30pm or once all registered classes are finished. To be eligible for the 15% discount 5:30 classes must occur on the same day as afterschool enrolment. 2C. 5:30 classes are registered monthly instructional classes and are not part of the afterschool program. Every effort must be made to attend each week. Pick up is at 6:30pm (no earlier, no later). Students do not need to be signed out of 5:30 pm classes. 3. Please pick your child(ren) up inside the studio. Children must be singed-out by the parent/guardian or person denoted on the registration/emergency contact form. 4. Parking/Drop-off I. Please limit stay in designated parking spaces. Street parking is available on Rosewood Ave and Lakeshore Blvd. II. Do not use MIDAS or SCOTIABANK parking spaces III. Please do not block parking spaces or driveway for any reason Tuition and Fees 1. Registration fee per family are due at the time of registration. 1A. Registration fees are non-refundable and non-transferable. 2. A non-refundable deposit or the first full month payment is due at the time of registration to secure and confirm registration in the program. 3. Fees are to be paid in full prior to the child entering the studio/class space. Minimum tuition amount (see section 3A) must be paid in full according to the payment schedule per day that the child(ren) are registered for. A child will not be allowed in the studio or to participate in the program or class if the tuition payment is outstanding. 3A. Tuition and fees hereby referred to in this document represent introductory pricing. Policies and fees and are subject to changes at any time with notice. 3B. Monthly fees are due on the 1st of every month (or earliest business day after the 1st) for the same month. (ie. Payment on September 1st is for the month of September) 3C. Program payment must follow the minimum payment schedule as outlined: i. Registration and deposit or first full month payment are due at the time of registration to secure registration in the program. ii. Deposit will be applied to the first monthly payment. i. Monthly payments are based on the daily rate as outlined out in the fee schedule including any purchased add-ons. ii. Monthly denotes days per calendar month. iii. Payment schedule per month includes only instructional school days as determined by the school or Board of Education. It does not include Professional Activity (PA) days, statutory or public holidays, school holidays or breaks or any other days the school or Board of Education has deemed schools closed for regular classes. 3. Discounts and Promotions: I. Afterschool program fee schedule includes a discounted rate based on per day(s) of confirmed registration. No further discounts will apply to multiple day registrations. II. Multiple Registration Discount: 15% discount applied to the subsequent registration(s) of the same program after the first completed registration paid in full. (ex. Afterschool Child 1 - Regular rate, Afterschool Child 2 - 15% off, Afterschool Child 3 - 15% off, etc.) *Multiple Registration Discount is applied to the already discounted rates as outlined in the Afterschool Program fee schedule. III. Any promotion or special offer is subject to change or expire at the discretion of the Directors with written 30 days notice to the registrant. 4. Refunds and Credits: Registration fees are non-refundable. Deposits are non-refundable. No refunds for registered days unattended. A refund/credit will be considered when a doctor's note is presented and is at the discretion of the directors. Refunds are non-transferable. Credits are non-transferable outside of immediate family and expire after one calendar year. Credits may be used towards any other product or service offered or provided by The School. Cancellation Policy 1. Withdrawal from the program is considered with 30 days written notice. With receipt of written notice of withdrawal the last day of participation in the program will be 30 days from the day the written notice was confirmed received. The client/registrants account will be billed according to that 30 day period for the days the program is the scheduled. 2. Spaces are limited. Once a withdrawal has been made from the program the clients understand that they are foregoing their spot in the program and could be placed on a waiting list should they decided to re-register. Code of Conduct 1. Respect. I. In this space we are compassionate learners. We respect each other and the diversity that makes each person unique. No tolerance will be given to any person that causes harm verbally, physically or emotionally by any means including off site yet linked to The School (IE: via social media, slander, gossip, etc) to any other member of our community. II. This is a shared space. This is your space. Please treat this space as you would your home. Your time spent at The School should feel calm, comfortable and stress-free. Kindly let us know if you notice an area that needs attention. III. Personal property and privacy. We encourage labelling all of your children's items that they bring with them to The School. Kindly leave anything that is extremely valuable at home. Please be aware when taking pictures of your child in the shared (lobby) space that no other child is visible in the photo. NO photos may be taken during a registered class or in the drop-in play area. Consent* Auto Billing AgreementThis Automatic Recurring Payment Agreement (the “Agreement”) is a contract between you and HAAN Artistic Group Ltd. of Mississauga ON, to set up your automatic recurring payment. Please verify that all information provided is complete and accurate before signing your agreement. Frequency of Payments An automatic recurring payment will be made each months on the 1st o the month for 10 months, or prorated from the time of enrollment. A registration fee of $30 plus any applicable taxes will be assessed with your initial payment only. You will not receive any advance notice of this payment, but you will receive a post payment confirmation email following all automatic recurring payments. Automatic Recurring Payment Failure If your automatic recurring payment is declined or unable to be processed for any reason, you will be contacted by phone or email. We may in our sole discretion permanently restrict your ability to use a certain payment method if that payment method fails. In the event any payment is not made and you do not make the required payment by the end of your billing cycle, your service will be suspended or cancelled. Changing Automatic Recurring Payment If you would like to change your automatic recurring payment method including, for example, making a change from one credit card that was initially selected to another credit card, you may do so by logging into your account. AGREEING TO THESE CONDITIONS By providing my credit (“Payment Method”), I AGREE that I have read and understand this Agreement. In addition, I authorize The School (HAAN Artistic Group Ltd) to charge the full amount required by my payment plan (initial reg fee + 1x per month payment) to the specified Payment Method; and I authorize the financial institution for the Payment Method specified above to charge my account and remit payment for my service to The School (HAAN Artistic Group ltd.) This authority will remain in effect until I give notification in advance of 30 days as required under this Agreement, to terminate this authorization; provided, however, that no termination of authorization will relieve me of any obligation to pay amounts due for services I have already attended.
